Checklist for Applicants:
o Application form ( pages)
o 6 Recent photographs
o Attested photo copy of birth certificate
o Attested photo copy of passport of applicant
o Attested photo copy of birth certificate or passport of father and mother of the applicant
o Original School leaving certificate of last school attended
o Student Medical History form
o Statement of Understanding (Rules and regulations)
o Official transcript or photo copies of report cards showing clearly grades or marks received during the last 2     years, as well as any standardized test scores that are available.
